Assumption of Risk & Release. Football and speed training involve physical activity and inherent risks including, without limitation, muscle strains, sprains, fractures, and contact injuries. By agreeing, I acknowledge these risks and voluntarily assume them on behalf of the athlete named above.
I release and hold harmless Coach Trent Trammell and Coach Trammell Speed & Performance, and their staff, from any claim arising out of participation, except for gross negligence or willful misconduct. I confirm the athlete is in adequate physical condition to participate.
I authorize Coach Trammell to seek emergency medical treatment if needed and agree I am responsible for any resulting costs.
